This book provides instruction in applying basic treatment strategies to patient care. Emphasis is placed on teachi. ng diagnostic skills, and especially, the therapeutic management of emotionally disturbed patients. This book is intended as a companion text to Basic Psychopathology: A Programmed Text, whi. ch focuses more on teaching the description and observational skills for diagnosing common psychopathologic syn- dromes. Basic Psychotherapeutics gives pragmatic suggestions for treating a variety of psychopathologic disorders, many of which are newly classi- fied in the Diagnostic Statistical Manual - III (DSM-II1) published by The American Psychiatric Association. We have adopted the terminology and many, but not all, of the diagnosti. c criteria used in DSM-III. The treatment recommendations proposed are eclectic in nature and the infor- mation presented is non-theoretical and patient centered. Basic Psychotherapeutics is divided into three parts. The first re- views basic data collection procedures and treatment skills; it includes chapters on interviewing, the mental status examination and pharmaco- logic, psychologic and behavioral therapy. The next section includes chapters on commonly encountered psychiatric syndromes such as schiz- ophrenic, affective and anxiety disorders. The final chapters are pre- sentations of special clinical interest, such as the management of suicidal patients, psychiatric emergencies, treating the dying patient and his family, and others.

This book, which proposes a developmental view of short-term inpatient treatment for severely disturbed children, is much needed in our field. It is particularly relevant, emotionally engaging, and a pleasure to read because the writers who are the actual participants and leaders in the milieu program, discuss their own experiences with a variety of children. The principles of milieu therapy are beautifully described and its application to a diverse ethnic population of sick children is clearly delineated. Although Sullivan's achievements have placed him in the first rank of psychiatry, some of the work done since he died in 1949 can be assimilated to enchance his achievements. For this reason, I enlisted the aid of Menachem Melinek, M.D., whose wide knowledge of re­ cent and contemporary psychiatric studies is admirably suited to the task of assimilating some of them to Sullivan's theories. psychiatric treatment approaches there are opportunities for trial and error, exploration and reconsideration, revision of treatment approach, and correc tion of errors. Revisions and corrections are based on observations of pro gress and on response from patients about the impact of treatment efforts. But emergency interventions usually are one time efforts involving a sequence of evaluation, therapeutic intervention, and referral out. Response is limited or absent and there are no opportunities for corrections over time. Therefore, interventions and referrals must be made on the basis of first evaluations, with positive conviction related to current best effort and despite the lack of guidance derived from tracking the patient's progress over time. The staff must tolerate the risk-taking required, and must also be prepared to forego the gratification of seeing the beneficial results of the work done. Even in pro grams structured to allow return visits, and despite the occasional information provided by other agencies after referrals, the majority of patients are lost to follow up. The usefulness of specific interventions is hard to evaluate and the satisfaction of seeing a task completed is rarely available. The emergency program staff must tolerate a considerable amount of stress in the environment. Emergency care centers are collection points for an extraordinary volume of human suffering. The anxieties and strains asso ciated with persistent urgency of need, alarm related to unexpected difficul ties, fearfulness about outcome, closeness of death, and presence of pain pervade the atmosphere.

Phenomenology and Treatment of Psychiatric Emergencies

This volume was inspired by an annual meeting of the American Col lege of N europsychopharmacology held in Maui, Hawaii. A panel on psychobiological issues of childhood was held, with presentations devoted to antidepressant drug levels in depressed prepubertal children, re sponses of normal and hyperactive children to stimulant medication, and the vulnerability of the adolescent offspring of manic-depressive parents to affective illnesses. The session drew a large crowd, and it seemed appropriate to develop these topics in a book. Many of the authors in this volume attended that conference, and the book reflects the fact that psychobiological research in children has moved even further along than was envisioned at Maui. In keeping with developments in the field, this volume surveys key topics of interest, including nosological issues surrounding the attention deficit disorder of childhood, the ontology of neurotransmitter systems in the human brain, and the relation between child psychiatric disorders and sleep patterns. Other studies link the clinical effects of drugs with plasma DBH activity or with attentional measures. The side effects of on growth are examined, as well as the ethical issues involved in drugs on children. These areas continue to be of vital interest.

Reproduction and fetal development in humans and experimental animals may be adversely affected by a wide range of drugs and environmental agents. They may be toxic in the parental generation leading to impaired germ cell formation, loss of reproductive poten tial and infertility following chronic exposure. Deviant fetal development occurs as a consequence of cellular damage at sensi tive stages in development. Changes seen range from intrauterine mortality and structural malformation to growth retardation with physiological and behavioural defects. Concepts on the mechanism of action of teratogens are discussed with reference to some better-known agents. Presently, regulatory authorities prefer whole animal studies in predictive safety evaluation of substances to which a pregnant woman may be exposed. Tests are conducted over one or more generations and are designed to study the influence of test com pounds upon general reproductive performance, fertility, fetal development and perinatal and postnatal behaviour. Experiments are designed to permit compounds to be tested under conditions resembling expected human exposure. Small rodents and rabbits are preferred species on account of their convenience as laboratory animals and the available knowledge regarding their reproductive behaviour, fetal development and known sensitivity to human teratogens.
