Infertility is a problem for the couple and evaluation of the couple is preferable in a clinic where both males and females may be seen together. The contribution to care by the urologist and the gynaecologist each with an interest in infertility is different but complementary. My appreciation of this was sharpened between 1976 and 1985 during my association with the World Health Organization's Spe cial Programme of Research Development and Research Training in Human Reproduction at the meetings of the Steering Committee of its Task Force on Infertility. The deliberations of this group were aimed at developing a protocol for investigation of the infertile couple and it became apparent to me as a gynaecologist how important is the precise characterization of the male partner's role to the formulation of the management plan for the infertile couple. To improve the quality of our evaluation of the male partner our techni cal staff were trained in the semen methodology of the laboratory of Dr F Com haire of Ghent and we were joined by Dr CLR Barratt, male reproductive biologist. It seemed appropriate to share our enthusiasm for clinical andrology, so after discussion with Dr Comhaire and Professor T Glover, an eminent male reproductive biologist, then of Brisbane, who planned a sabbatical to his native was arranged for 1 and 2 May 1986. Three points must strike anyone who has embarked on a study of dementia over a period of time. Firstly, that our conception of the syndrome is in a state of flux. Gone, for instance, in the past decade or two, is the requirement of a chronic, progressive, irreversible disorder for the diag­ nosis. I remember the surgeon who, when I was a student, returned a referral saying he would operate on the man when his dementia got better. Feeling superior, and encouraged by the consultant psychiatrist, we students laughed a good deal at this. Before we finished clerking on that Unit a visiting Professor of Psychiatry had demonstrated the reversibility of the symptoms of dementia in a patient with a rare metabolic disorder. Perhaps ignorance is sometimes an advance on received wisdom. The lesson is the concept of dementia must always reflect the state of knowledge and is therefore in a sense ad hoc. Secondly, what the criteria for, and also who the arbiters of, the diagnosis might be is not always clear. It is traditional to think that expressing opinions and making diagnosis of mental illness is almost a civic right, i.e.

The appreciation of risk like the awareness of beauty lies very much in the eyes of the beholder. It involves a value judgement and can never be absolute. Yet paradoxically, modern society is demanding ever greater degrees of safety in the medicines it takes, to the extent that nothing short of the total absence of risk will be tolerated. Since 1960, and mainly as a result of the thalidomide tragedy, governmental regulation of testing and use of new medicines has grown apace throughout the world. It has derived impetus not only from the understandable wish of the public to seek protection, but also from the anxiety of bureaucrats and politicians not to be seen to have made mistakes. Both these concerns have been inflamed by the recognition of the media that all drugs make news and horror drugs make the best news of all. Prior to this time the physician and his cures enjoyed a relatively supportive public. It was true that quacks existed and were recognized as such but, in the main, people wanted to take medicines and expected them to do them good.

There are few publications indeed that provide a completely reliable companion to bedside and out-patient instruction for the medical student - and even fewer that enhance the established physician's techniques. Three International Congresses on the Menopause have been held during the last five years, evidence of the explosive increase in the scient ific study of this subject. The first, held in La Grande Motte, near Montpellier, France, inJune 1976, was designed to provide a consensus on menopause research (van Keep et al., 1976). The second, held in Jerusalem, was planned to assess the developing research (van Keep et al., 1979). The objective of the third congress, convened inJune 1981 in Ostend, Belgium, under the auspices of the International Menopause Society, was to explore areas of controversy in basic understanding and in the therapeutic advances relating to the climacteric. The workshop moderators were selected because of their acknow ledged leadership in menopause research, and each was invited to arrange a workshop on a specific area of interest or concern. The moderators, whilst being given a free hand as far as the selection of invited speakers was concerned, were asked to allow plenty of time for open discussion. In particular, they were encouraged to stimulate debate about controversial issues.

The original Beecham Manual for General Practice was produced by Dr Selwyn Carson, of Christchurch, New Zealand, whose objective was a set of instructions for patient care for his practice team. Beecham Research Laboratories published and distributed it. Dr Ed Gawthorn of Melbourne, Australia, edited an Australian version again published and distributed by Beecham Research Laboratories. We were invited to adapt the New Zealand and Australian editions for British readers -but we decided that we should produce an entirely new Manual. This was done and it was published and distributed to general practitioners by Beecham Research Laboratories. This latest edition has been revised and updated. The Manual is a ready reference on planned care of certain age groups and situations; specific procedures and emergencies; and clinical care in general practice of important conditions.
